Client Information

Services offered for clients 13 years of age and up.  

Professionalism and Ethics

This office is committed to providing the best possible professional care for its clients. Consequently, the therapist is a licensed mental health professional.  This license requires the therapist to complete 15 hours of continuing education hours per year, and make a commitment to ongoing personal, professional assessment and growth. 

 

The therapists and business staff of this office adheres to the statutes of the State of Tennessee, applicable Federal laws, respective discipline ethics, and the National Association of Social Workers Code of Ethics.

 

Client Responsibilities

To maximize the benefits of psychotherapy, it is necessary for each client to abide by the following guidelines:

 

  • Every client is required to treat all office staff with dignity and respect.

  • Every client is encouraged to provide accurate information.

  • Every client is encouraged to comply with the provider's recommendations for treatment and to discuss concerns with therapist.

  • Every client is expected not to engage in illegal acts, such as forging or falsifying a provider's name on documents requiring a provider's signaure.

  • Every client is required to pay any fees at the time of the appointment.

  • Every client is requested to keep scheduled appointments or to notify the office if he/she decides to discontinue treatment.

Insurance/Payment

 

As a courtesy, we will file claims with the insurances in which we participate.  Clients will be responsible for any payment not covered by insurance carrier.  We accept cash, check, debit or credit.  The office will be happy to provide the client with a receipt if he/she would like to file their own insurance claim.

 

We participate with the following insurance companies:

  • Blue Cross/Blue Shield

  • Cigna

  • Aetna

  • Magellan

  • TennCare

  • TennCare Cover Kids

  • Medicare

  • the majority of Employee Assistance programs

 

 

Court Appearances/
Subpoenas
There will be a $100 per hour charge for any court appearance.  This per hour charge will not only include time spent in the court room, but any preparation time (i.e. research, etc.)

Cancellations/

Missed Appointments

 

 

 

Cancellations MUST be made 24 hours in advance of the scheduled appointment.  Failure to comply with this cancellation time frame or missing a scheduled appointment will result in a $50 charge for the appointment.

 

Insurance will not pay for missed appointments.

 

Consequently, payment for the scheduled service ($60) must be paid in full prior to scheduling and/or being seen for your next appointment.

 

If we must cancel a scheduled appointment, we will give you as much notice as possible, and will make every effort to reschedule for a time that is convenient for you.

Medical Records

Copies of medical records, for you or your dependent, must be requested in writing and paid for in advance.  The charge is $20 for pages 1-25, then $1 for each additional sheet.  Please allow 10-30 days for processing.
